Thyroid Tuberculosis



Abstract
The thyroid tuberculosis is very rare disease, and occationally reported in Korea, but the differential diagnosis and its method are difficult problem, yet. The tuberculosis is spreaded to any organs, and more affected to respiratory tract, lymphatic
system, kidney, bone and G-1 tract, and less to thyroid, pancreas, spleen and striated muscles. Since 100 years ago, about 100 cases were reported in Euro-American area, and its incidence was about 0. 1% of all thyroid disorders proved by tissue biopsy.
The patient was 37 years old housewife, and suffered from the intermittened mild fever, swallowing difficulty and swelling on left thyroid region.
Until 7 months prior to admission, the patient felt the palpated multiple lymphatic nodules in right subclavian region and intermittened mild fever and coughing without sputum, too.
Then since 4 months ago, the diffuse fixed mass was noted on left thyroid lobe which is not hotness or flucctation, and accompanied the swallowing difficulty.
During that time the patient was treated with anti-Tuberclous drugs, but the symptoms were not i improved. Then the thyroid scanning was taken, and revealed the diffuse cold area with hypothyroidism pattern, and suggested the cold abscess.
The lymph nodes on right subclavian area were reported as the tuberculosis lvmphadenitis, and the operation was taken under impression of tumor. But there was not any abscess lesions and the cut margin was revealed the nodular lesion, and left thyroid lobe was diffusely enlarged, and the regional I lymph nodes were, too.
Microscopically, there was noted the giant cells, epitheloid cells and caseous necrosis surround the thyroid follicles, and Ziehl-Neeison stain was noted the acid fast bacilli, and confirmed the tuberculous thyroiditis, pathologically.
A case of thyroid tuberculosis associated with regional lymphadenitis, admitted to St. Mary hospital, Catholic Medical College, is reported with literature review.
